Original front and reverse draft registration card inspected October 3, 2026 in FamilySearch, Illinois WWII Draft Registration Cards, image 1069 of 1071; indexed record QPZH-3G67. Sam Monasteri, born September 1, 1922 Chicago; registered June 27, 1942, local board 39; employer Chicago Screw Co; mother Mrs Phylis Monasteri. These birth/name/place details correlate with Sam/Salvatore in source 300.10.046. A handwritten annotation across the top reads “discharged Jan. 18, 1946.” This annotation is present in the original scan but absent from the displayed index transcription. It supports a January 18, 1946 discharge-date lead; writer and date of annotation are unknown, and the card is not a separation certificate. Navy service during WWII is separately reported in Sam’s 2009 obituary (300.10.046); the card annotation does not name a branch, ship, unit, rank or theater. Registration serial 126 and order number 11412 are draft-board numbers, NOT a military service serial. Seek Navy separation/personnel record to confirm date and assignments. VA VLM surname search did not return Sam; this does not contradict service. Screenshot of original attached.
